Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Important information we will gather at this time includes your medical history, a brief physical exam, reason for your visit and any radiographs necessary. The doctor will then review this information and recommend a treatment. He will also explain the treatment in detail, because understanding the treatment is important in making you comfortable with your procedure. Your consultation visit will conclude with a brief visit with one of our financial coordinators. They will go over the proposed treatment plan and the associated fees and find a convenient time to schedule your surgery.

Your personal and insurance information and a brief health history can be filled out prior to your visit. Simply go to Online Registration page and fill out the form. You are not required to sign or print the form. When finished, you need only to click the SUBMIT button at the bottom. Your information is then submitted to our office through a secure internet portal guaranteeing your information will remain confidential.If you have dental or medical insurance which you will be using for your visit and treatment, please bring your cards with you so we may make copies of them.
