Your insurance contract is an agreement between you and your insurance carrier. We strongly encourage patients to study the provisions of their insurance prior to surgery as there is significant variation in coverage between companies and plans. We are pleased to seek pre-treatment authorization on request following examination and consultation.
As a service to you, we will file claims with your insurance carrier. We require a deposit, as well as deductibles and non-covered services to be paid on the day of service. For your convenience we accept cash, checks, and the following credit cards: Visa, Mastercard, and Discover.
Balances remaining following insurance reimbursement are your responsibility. Feel free to contact our Billing Department if you have any questions prior to your treatment.
